Synth Health Guardrails — consensus-label release

Private release candidate. Licensed under CDLA-Permissive-2.0.
The repository remains private until League Labs publishes the release.

Sensitive-content notice. Although every record is synthetic, this dataset
intentionally contains realistic-looking medical crises, synthetic PHI/PII,
adversarial instructions, jailbreaks, prompt injections, and hostile language.

Synth Health Guardrails contains 13,713 synthetic member messages for
training, calibrating, and evaluating healthcare input-guardrail classifiers.
Every provided safety label is an exact blind consensus between
Gemini 3.6 Flash and Claude Opus 4.8 on Vertex AI across category,
severity, and routing decision.

This is model-based adjudication—not clinician annotation, legal review, or human
ground truth. Consensus reduces dependence on one judge but can preserve shared
rubric bias and correlated model errors.

At a glance

Question Answer
Contains real member, customer, or production data? No. Every record is synthetic. No production system, customer environment, or real traffic is used or sampled.
Contains real PHI / PII? No. Records describe fictitious people. Content is deliberately sensitive-shaped by design — see Risks.
Label source Exact blind two-model consensus (Gemini 3.6 Flash + Claude Opus 4.8), not human annotation.
Uses third-party data? Aggregate, public-domain statistics from CDC PLACES, used only to weight how often health topics appear. No PLACES records are included.
Attribution required? Yes — CDC attribution + non-endorsement (see Population prior & attribution).
License CDLA-Permissive-2.0. Commercial use is permitted; the license text must accompany shared Data.
Clinical ground truth or medical advice? No — out of scope.

What makes this release different

  • SYNTH context is first-class data. Every row retains the structured trait
    snapshot that conditioned the message, including demographic, health-literacy,
    technology-comfort, privacy, trust, risk, communication, care-navigation, current
    situation, and trajectory fields when available.
  • Labels are auditable. labels contains the released consensus judgment;
    label_adjudication retains the original generation target and both blind model
    judgments, rationales, confidence, and ambiguity signals.
  • Text lineage is preserved. Original text, final text, complexification
    attempts, critic decisions, source model/row IDs, and recipe versions remain
    attached. Adjudication did not rewrite text or traits.
  • Non-consensus rows are excluded. The parent RC contained 19,478 rows.
    This release retains only the 13,713 rows with exact full-contract
    Gemini/Opus agreement.

Quick start

Pin the dataset revision (the commit hash shown on the Hub for this release)
when comparing models:

from datasets import load_dataset

dataset = load_dataset(
    "LeagueLabs/synth-health-guardrails",
    revision="ee7b400eec9e45da685d333ebb7c822b6702b1c2",
    token=True,
)

train = dataset["train"]                # model fitting
calib = dataset["dev_calibration"]      # threshold selection only
test = dataset["test_public"]           # held-out general evaluation
red_team = dataset["test_red_team"]     # held-out adversarial evaluation

# The 3-way routing decision is deterministic given the 8-way category:
DECISION_OF_CATEGORY = {
    "clean": "allow", "off_topic": "allow", "phi_pii": "allow",
    "emergency_detection": "escalate", "escalation": "escalate",
    "jailbreak": "refuse", "prompt_injection": "refuse", "toxicity": "refuse",
}

Do not tune on either test split. Preserve row IDs, preprocessing, and decision
mapping when comparing systems.

Splits

Split membership was inherited from the frozen parent RC; rows were filtered in
place and were not re-split after adjudication.

Split Rows Intended role
train 9,887 Model fitting
dev_calibration 680 Threshold selection and calibration
test_public 2,050 Held-out general evaluation
test_red_team 1,096 Held-out adversarial evaluation
Total 13,713

test_red_team is deliberately concentrated on adversarial categories
(jailbreak, prompt injection, PHI-elicitation, toxicity) to measure
generalization to unseen attacks; it is not distribution-matched to train.

Released label contract

Each row has one category, one severity, and one deterministic routing decision.

Category Decision Operational meaning
clean allow Ordinary healthcare navigation request
off_topic allow Request outside the healthcare-navigation scope
phi_pii allow Synthetic sensitive identifiers are present
emergency_detection escalate Possible immediate safety or medical emergency
escalation escalate Explicit non-emergency human handoff or complaint
jailbreak refuse Attempt to override role or safety behavior
prompt_injection refuse Instruction or data payload targeting hidden/system behavior
toxicity refuse Abusive, hateful, threatening, or severely hostile content

Severity is low, medium, high, or critical. It measures the strength and
consequence of the guardrail signal under this dataset’s rubric; it is not a
clinical diagnosis or a general-purpose measure of medical acuity.

How it was built

  1. Generation. League Labs SYNTH generated provenance-preserving candidate
    cases from structured personas and situations, using two source pools —
    Cohere Command A+ (05-2026, W4A4): 6,748 rows; NVIDIA Nemotron 3 Super 120B (A12B, FP8): 6,965 rows. Each row carries its safety contract and
    generation receipts.
  2. Curation. Candidates passed validation, exact/normalized lexical
    deduplication, BGE-M3 semantic deduplication, targeted complexification of
    redundant/low-information cases, and independent critic checks.
  3. Blind labeling. Gemini 3.6 Flash independently labeled each final member
    message without seeing its source label, source-model identity, or SYNTH trait
    snapshot.
  4. Blind adjudication. Claude Opus 4.8 independently adjudicated under the
    same blind rubric.
  5. Consensus selection. This Hub release retains only rows where both models
    returned the same category, severity, and decision.

The two judgments were produced independently, but both are foundation models.
label_adjudication should be treated as model-agreement evidence, not human
inter-annotator agreement.

Consensus statistics

Item Value
Parent RC rows evaluated 19,478
Exact full-contract consensus rows retained 13,713 (70.4%)
Rows excluded because Gemini and Opus disagreed 5,765
Consensus labels differing from the generation target 9,176
Consensus labels confirming the generation target 4,537
Text or trait changes during adjudication 0

The original generation target is provenance, not gold truth. For this release,
the exact two-model consensus is promoted to the provided label contract.

Row schema

Field Description

Traits are not inferred when source provenance is absent; missing values remain
explicit rather than being invented.

Intended use

  • Training and calibrating healthcare input-guardrail classifiers.
  • Evaluating category, severity, and allow/refuse/escalate routing.
  • Testing robustness across synthetic communication, literacy, privacy, trust,
    care-navigation, and situation variation.
  • Reproducible research on synthetic safety-data generation and model adjudication.

Out-of-scope use

  • Clinical diagnosis, treatment, triage, or medical decision-making.
  • Treating model consensus as clinician-reviewed ground truth.
  • Inferring facts about real people, League members, or a real population.
  • Re-identification or attempts to map synthetic personas to individuals.
  • Training a general dialogue model directly on harmful examples without a
    separate safety design and evaluation.
  • Treating intended-use guidance as a substitute for application-level safety
    controls, validation, or applicable healthcare obligations.

Risks and limitations

  • Consensus selection bias: difficult and ambiguous rows are underrepresented
    because non-consensus samples were excluded (29.6% of the
    parent RC did not reach exact consensus).
  • Correlated judge errors: Gemini and Opus can share blind spots or interpret
    the rubric similarly without being correct.
  • Synthetic realism: plausible wording does not make a case clinically factual
    or population-representative.
  • Severity sensitivity: severity was the least stable field in the parent RC;
    downstream users should calibrate it separately from category and decision.
  • Boundary coverage: use the excluded internal review cohort—not this
    high-consensus subset—when specifically studying adjudicator disagreement.
  • Sensitive-shaped data: PHI/PII-like values and crises are fabricated, but the
    material can still be upsetting or trigger content filters.

Population prior & attribution (CDC PLACES)

Health-topic frequency in the synthetic population is weighted using aggregate,
public-domain statistics from CDC PLACES: Local Data for Better Health
